| 内容記述 | 現在の医療制が確定した1983年の老人保険法成立以後, 今日までの日本の医療政策を公共経済学的視点から考察する．この期間を含めて戦後日本の医療政策は，社会保険制度の３主体の行動で特徴づけられる．過激な規制によって医療を計画経済的に資源分配してきた厚生省（当時），保険社機能が規制され無力化された保険者，市場でなく規制による資源配分の下で強力な政治者影響力を行使して rent-seeking を実現してきた供給者である．低い医療費と長い平均余命など日本の医療は表面的には高く評価されるが，競争誘因の欠如や資源配分の歪みなどによって，効率性や品質など改善するべき点が指摘される．急速な少子高齢化の進行により高齢者医療費の一層の上昇が予測され，マクロ経済との上均衡と現役世代の負担増による制度の破綻は必至で，規制緩和で競争誘因を高め，情報技術の導入や保険者機能の促進による，日本の医療改革が主張されている． | |||||||||

| 内容記述 | It is estimated the national health expenditure in 2025 may be about two times the current level, or a yearly increase of 2.8% in a Japan facing the rapid aging of society. We are concerned it may be very difficult for Japan to manage to maintain expensive health expenditures. Furthermore, the younger generation cannot afford such high levels of support for the elderly, and the present health system cannot be kept up in the near future. The three representative players that have performed in the history of Japanese health policy are bureaucrats with too many regulations, weak insurers and politically strong suppliers. We propose that health resources should be allocated by the market mechanism and not by regulations. | |||||||||
